Medical Plan Information
The Santa Monica UNITE HERE Health Benefit Fund provides eligible participants with the option to enroll in one of three different Plans: Martin Luther King Community Healthcare Plan, Kaiser or Health Net. Participants can review the Medical Plan Comparison shown below for information on each of the Medical Plans offered by the Fund.
An enrollment form is required for participants who select the Martin Luther King Community Healthcare Plan, Kaiser or Health Net Medical Plan.

- Martin Luther King Community Healthcare Program SBC (7/1/2024 – 2/29/2024)

Kaiser:
Health Net: (enter your location, then select "HMO-Salud HMO y Mas (with walk-in clinic large group/small group")
MLK:
Under certain circumstances, MLK members will use providers in the First Health network. Prior authorization is required to use the First Health network. To find a specific provider, or for a list of First Health providers, contact Member Services or the Fund.
In network pharmacies include CVS, Rite Aid, Ralph's, Albertson's, Safeway, and many others. To find all in-network pharmacies near you, log in and search: https://express-scripts.com/ DO NOT USE WALGREEN'S, AS IT IS NOT A NETWORK PHARMACY
